The Sarpy County Holiday Assistance Program will take applications on Nov. 5 from 9 a.m. to noon and 3 to 6 p.m. at Trinity Lutheran Church in Papillion, 330 W. Halleck St.
The program serves low-income Sarpy County residents with children from birth through age 12 with new Christmas toys.
Applicants must provide:
A photo ID for the head of household (person applying) and Social Security cards for everyone in the household. A tax form can be used in place of the Social Security cards.
Proof of ages for children age 12 or younger: birth certificates or current Medicaid card.
Proof of residency in Sarpy County such as a utility bill (OPPD, MUD, Black Hills Energy) or a letter from the state.
Proof of total household income, such as pay stubs for the last 30 days or an unemployment letter.
Please do not bring children to the sign-ups.
A second registration opportunity will be held Nov. 12 from 9 a.m. to noon and 2 to 6 p.m. at the First Baptist Church Gymnasium in Bellevue, 23rd Avenue and Hancock Street.
The assistance program is sponsored by the City of Bellevue’s Human Service Office, Heartland Family Service Sarpy Office, Sarpy County Human Services, Sarpy County Cooperative Head Start, Eastern Nebraska Community Action Partnership, Sarpy County Community Service and St. Columbkille Church.
